ABSTRACT: Introduction: Old age is characterized by the appearance of several complex health states that are usually known by the name of geriatric syndromes. These are based on multiple factors one of these syndromes is frailty; it can be considered as a progressive age-related deterioration of physiological systems leading to a decline in intrinsic capacity reserves, which confers extreme vulnerability to stressors and increases the risk of a number of adverse health outcomes. Objective: To determine the presence of frailty in a group of adults and older adults institutionalized in the Fundación Hogar Sara Zapata, in the municipality of La Mesa - Cundinamarca. Methodology: It is a mixed approach study with an interpretative-descriptive scope. The sociodemographic profile was characterized, and the presence of frailty and its associated factors were determined by means of the instrument: Fragility Scale Examination (Spanish version) which consists of 5 questions with an answer option each one with its respective score from 0 to 5. Results: Seventy-six institutionalized persons between 58 and 101 years of age participated. There were 34 women and 27 men in a ratio of 1.2:1 respectively. Regarding the results of the applicability of the frailty scale test, the older adults reported feeling tired at least some of the time in the last four weeks before taking the data for the present study. Forty-seven percent reported feeling tired most of the time and only 4% reported feeling tired some of the time. We checked whether older adults reported difficulty climbing 10 steps without rest and without some type of assistance, as well as whether they could walk hundreds of meters without assistance and found that 64% had difficulty climbing steps and 43% required assistance to walk hundreds of meters. The study participants reported that most have been diagnosed with hypertension, followed by COPD, DM, arthritis, among others, and only one older adult reported asthma as a diagnosis. Weight, height and BMI showed that there is no difference between older adults classified as frail or not frail, and when classified as frail or not frail, 52% are frail and 48% are not frail. Conclusion: The prevalence of frailty in the population studied was 52%. On the other hand, weight, height and IMC showed that there is no difference between older adults classified as frail or not frail, which means that the greater the number of diseases, the greater the tendency to suffer from frailty. The application of the instrument was carried out by a professional nurse from the foundation, thus giving the research a focus on the art of care. The importance of Dorothea Orem's systems theory was taken up again, through the socialization to the nursing staff of a primer type resource aimed at the results of the research, focused on the promotion of physical activity as an intervention proposal. The implementation of the booklet for the use of the foundation's nursing staff was agreed with the directives, generating a positive impact in the medium term in the adult and elderly of the place, measuring in the future the adherence and impact of this on the prevalence of frailty.es_CO
